Rich text editor improvements

Ultra Course View
We made some improvements to the rich text editor in the Ultra Course View:
- We moved the file attachment function to the top level so it's easier to access.
- We added a strikethrough font style option.
- We updated our function groupings so they're easier to read on smaller screens.
- We allowed the browser to control the background of highlighted text to improve accessibility.
